Linkage Trust,
A great resource for Health and Social Service Professionals and the Waikato Community. Linkage Trust is re-launching its Information Consultancy service under the Service Navigation brand.
Located at Unit B2 99 Greenwood St, Frankton, health and social service professionals are able to refer patients to this free confidential face to face service between the hours of 8.30am to 5.00pm Monday to Friday.
Self-referrals are also more than welcome.
People can either make a specific appointment with a Service Navigator or walk-ins are accepted.
The service is also available by phone 0800 WEBHEALTH (932 432) or online at http://www.webhealth.co.nz/waikato then select the ‘Service Navigation’ tab.

Service Navigation involves working one-on-one with people to assist them in accessing the information and services that best meet their needs. Linkage has four Service Navigators located at their Greenwood St office.
The process begins with identifying the person’s needs and identifying appropriate services. Contact with these services is then facilitated as appropriate. The client receives follow up contact to ascertain if needs have been met by the service.
If this is not the case alternative services or courses of action are recommended. Throughout the process the Webhealth website is used as an essential tool to locate appropriate services and information and clients are educated and encouraged to use Webhealth to find their own solutions for future issues they may have.
