Ministry of Health, National
A changing primary health care system requires a responsive and connected information environment that enables people to take informed action to improve health and well-being.
Primary health care participants include individuals, whānau, families, communities, health providers, District Health Boards (DHBs), primary health organisations (PHOs), non-government organisations (NGOs) and government agencies.
The information environment should also facilitate coordination of activities and collaboration between participants.
